A cognitive assessment is a professional determination of a person's health. This assessment generally happens as a part of the claims or application process for Long-Term Care Insurance. 

A nurse or a licensed health care practitioner would observe an individual's physical and mental health. They would look at their ability to perform everyday activities and cognitive functioning. 

This assessment, along with medical records and a doctor's statement, will help determine eligibility for a new Long-Term Care Insurance policy and available benefits during a claim.
